Reporting a Permanent Exclusion

Should it be necessary to permanently exclude a pupil from a Hinckley and Bosworth school, this form needs to be completed and sent on the same day to enable continuity of provision and information transfer.

School referral to HBEP

In order to make a referral to HBEP for part time or full time program management, complete the two steps below:

The online form will require you to upload the completed McSherry assessment and most recent SEND Support Plan or EHCP. If your pupil is CIN or CP you will also need to securely send via Egress their safeguarding chronology, which will be securely stored by HBEP.

Once a referral is received, you should expect contact from the Partnerhship Coordinator within two days to liaise with you to set up an admission meeting at the school.

Commissioning Agreement

This is a Commissioning Agreement between the school and HBEP and an agreement that the following will be in place ( Ref: DFE-300-2013 2016 Alternative Provision guidance) :

-Responsibility for the alternative provision used rests with the commissioner (in conjunction with HBEP who will program-manage this). The nature of the intervention, its objectives and the timeline to achieve these objectives should be agreed and clearly defined.

-Progress against these objectives should be frequently monitored, appropriate reviews should be built in and continuity into the next stage in the child’s life should be considered. School link will attend six-weekly reviews with HBEP and keep up to date with information shared through the Google Drive; link will act upon and pass on any information within school as necessary re attendance, SEND, safeguarding. School governing body will be made aware of all reviews completed.

-Where reintegration to the school is an objective, there should be agreement on how to assess when the pupil is ready to return and the school should provide an appropriate package of support to assist their reintegration. HBEP will use the McSherry Doyle RRS assessment to help with this process.

-These objectives and plans should be agreed with providers, set out in writing and regularly monitored, including through frequent visits to the provider. HBEP will pass on all referral information to providers who are commissioned including SEND and safeguarding information needed.

By completing this referral form you are agreeing to these statements.
